ISWI 2.03 Patch Wizard not always updating

I have gone through the patch wizard and the #### thing only updates a couple report files but not my exe or ocx and dlls. I have mapped the patch to the original version that I want to patch and the new version of the full install that I want to bring everyone up to. i have been very conscious of install GUIDs, versions, and file versions in my install and I believe that I have incremented everything correctly.  The GUID of the dll and the ocx files have not changed, so there is no fancy COM information to change.. Can someone give me some other things to check?

Also, while I have the attention, is there a way to supress a custom action in a patch that was run already by the original install?

You can give it a condition of Not PATCH

Every since I upgraded to 2.03 of installshied
for windows installer, I can't get a patch to work either.
I created a log file of the patch process,
but when I search msdn or the newsgroups with
the errors I get nothing.

I am getting Transforming table error.
