Anyone have an idea to read and write entries in a database of an ism file?

In the General project settings you can chose a project file format. By default it's using "binary" but you can change it to "XML". In XML format you can use a XML tool of your choice. When using Binary you can use the MSI functions, because internally the binary .ism format is identical to .msi (you can even open a .ism file in Orca), so you have all of the msi automation commands available.
Independent of the project file format setting you can use InstallShield's automation interface to modify your project.
