The problem is the registry keys need to be present before the application is launched.
Any ideas?

What events will cause the repair option to automatically trigger? I have tried reading missing registry entries but that appears to not work. The only way I have seen to to run the executable.

I am not sure. I think you need a component with a registry key as the key 'file' as a member of the feature that the shortcut to the target component is being run.
Alternately you can call MsiLocateComponent in your program to ensure the presence of the key and MsiProvideMissingCompoent to trigger the minimum repair if it is missing. If a component with only registry keys is created and is included in a feature with no file components a repair will not require the source which should keep your clients happy.
