On a clean install of NT 4 sp6, if I install my device driver I get the message "1058 service is disabled and cant be started".

The driver is not disabled, and its registry entry looks to be ok.

To get the system to work, I have to either run an old install I made with installshield professional 5.1, or run the numega monitor program, then delete and recreate the registry entry.  

When the system is working the device entry in the registry looks the same as when it wasn't working

There isn't a service that it needs

Check the security permissions of your registry entries!
To do that compare your permissions using the registry editor regedt32 Security->Permissions.
In my case the ALLUSERS property was not set to 1 and therefore all registry entries had the wrong permissions.
