Robert M. Dickau has described how to use a custom action "fix_reinstall" to handle both fresh installs and upgrades without changing the setup.ini file.
If the target computer has Windows Installer 2.0, this method doesn't work. No files are installed.
If the target computer has windows Installer 1.11, everything works normal.
Any ideas?

OK, so I found it myself.
After re-reading the article a little more thoroughly I discovered it had been updated since last time I read it.
After having inserted the fix_reinstall action in both User Interface and Execute sequences it now works as expected.
